EHR vendors claim integration is simple. But in practice, it quickly becomes a long process of tickets, approvals, and testing cycles, with unexpected issues popping up beyond the original project scope. A recent project across several clinics proved this point again. It’s worth sharing, since these same issues keep coming up.

The System Architecture Seemed Solid, but the Data Was Another Story
HL7 and FHIR were set up. Governance was documented. The technical foundation was solid. Still, the project stalled, not because of connectivity, but because of issues with meaning.
Weight was recorded in different fields depending on the site. Sex was stored in various columns and formats across departments. Workflow customization was so extensive that two sites using the same Epic version acted like completely different systems. Every small change to the Epic environment required approval and testing, adding months to already tight timelines.
These issues, combined, create downstream inconsistencies that break dashboards, corrupt analytics, and undermine automation before deployment.
The Real Blocker Isn’t the Interface. It’s the Process Behind It
The hardest part of EHR integration isn’t moving data. It’s about ensuring the data means the same thing across teams, sites, and workflows, and that it was captured consistently enough to trust later.
Standards help everyone speak the same language, but they don’t remove complexity when each clinic has its own workflows and governance. The standard sets the framework, but not the details inside.

- Map the clinical workflow before you map the interface.
Don’t begin with HL7 message specs or FHIR resources. Instead, look at how data is actually entered at intake. For example, see how a nurse records a patient’s weight, how a physician might correct that entry during a follow-up, or how registration staff deals with missing demographic data. Think about who enters the information, when corrections happen, and how lab results are reviewed and shared between departments. What happens if data is missing or entered inconsistently? Knowing these details will strongly influence your integration approach.
- Define a minimum data quality contract before building pipelines.
Agree in writing, across teams, on which fields are critical, what values are allowed, which system is the source of truth, and how exceptions will be handled. This conversation can be uncomfortable, but it is the one that prevents the most rework.
- Assume same-EHR integrations will still be different.
Two clinics using the same platform can still have very different workflow customizations, local governance, and data entry habits. Treat each site as a separate integration unless you know for sure they are the same. Assuming they are identical is often where scope creep begins.
- Treat Epic’s approval process as part of delivery, not a delay.
Epic’s change management and validation steps are there for good reasons. They are required, and they take time. Include these timelines in your delivery plan from the start. Teams that treat them as surprises lose months, while teams that plan for them only lose days.
- Measure data inconsistency early – upstream, not after go-live.
If key fields differ by site, specialty, or user habits, those differences will show up in the interface. It’s better to find these issues during design than after a failed analytics rollout. Checking data quality before building pipelines isn’t extra work—it’s insurance.
- Make integration ownership cross-functional.
IT can build the interface, but it can’t fix the process that leads to bad data. Clinical operations, informatics, compliance, and frontline leaders all play a role in making integration work. If these stakeholders aren’t involved during design, you’ll need them during remediation, which is a much harder time to bring them in.
What This Means for AI, Automation, and Analytics
The impact of this is significant. Most MSOs and health systems are being offered AI-assisted workflows, predictive analytics, and revenue cycle automation. These tools are real and effective, but only if the data supporting them is clean and consistent enough to produce reliable results.
If your EHR integration layer is fragmented, your data pipeline will be fragmented too. A model trained or run on fragmented data won’t give you real insights. Instead, it produces results that sound confident but are actually unreliable.
Integration work isn’t just a boring step before the interesting parts. It’s the foundation that decides whether those exciting features will work at all.
